• Reproduce or identify the interaction failure cause in the approve/deny ban handlers
• Make the smallest possible code change to fix the interaction flow
• Add or update focused tests for the changed behavior if existing test infrastructure covers it
• Run targeted validation for the affected code paths
• Run code review and security checks, then summarize the fix
